When an Axis Fault (flagged as SOAP 1.2) is converted into a SOAPEnvelope if
there are more than one SubCode, only the last subcode is added to the
envelope....rather each subcode overwrites any subcode set before it.
See [AXIS2-4216|https://issues.apache.org/jira/browse/AXIS2-4216] from
2009-2012. This old ticket directly addresses this problem, provided a patch,
and according to the comments the patch was implemented, so I am assuming there
may have been a regression here?
In theory, each SubCode should be added as a child of the last processed
SubCode - nested structure.
