This patch fixes the problem by adding the reference parameters from the WSDL 
to the EPR returned in BindingProvider.getEndpointReference and checking if the 
reference parameter already exists in AddressingOutHandler before adding it 
from the WSDL.  This allows the values set on the EPR passed
to Service.getPort to override the values in the WSDL.
