This is a very generic error and is basically equivalent to a http 404 error.
Lots of things can cause this, the possibilities are almost endless. If its
intermittent then I would suspect a network issue. An overloaded proxy server
for example could be the culprit.
You don't mention your version of Axis2, does the error still happen with 1.7.8?
