Has the broad stokes - SpanScorer becomes QueryScorer, QueryScorer becomes 
QueryTermScorer, and QueryScorer gets a setTokenStream rather than passing it 
in the constructor.

Not sure how to best preserve any history here since SpanScorer is moving to 
QueryScorer.
