The migration guide that came out with 4.0 is probably the best place to start.
http://lucene.apache.org/core/4_8_1/MIGRATE.html is from the current release but probably hasn't changed since 4.0. There's also the changes file with every release. And if you browse the list archives I expect you'll find similar questions and answers. -- Ian.
