I've noticed something similar - 'there is currently no item in focus' - when I close a program and I'm not on the desktop (or on the Taskbar); but only since last summer when I upgraded to Windows XP on JAWS7.0. However, I haven't tested with another screen reader to ascertain if it's a JAWS-specific thing.
