Mark, I have been looking at this problem for you, trying to understand why 
your system won't acknowledge the pitch increase.  If this sounds silly, 
then please disregard.  But when you are entering the Jaws dialog, it often 
gives you 2 choices to make, either the Jaws default or the program's 
specific settings.  Is it possible that you are switching from one to the 
other without knowing it?
For example, if you are in Outlook and you haven't saved the configuration, 
the default is used.  But if the problem exists in Word, there may be a 
separate configuration which has been saved.  If you were to then make a 
change in the default, it would not effect the program specific settings.
