This happens to me too. Only ever seen it in Windows 10. Somehow, although this seems unlikely, I think this has something to do with JAWS. The windows are all still there. A sighted person can use the mouse to get to them, and of course they are still on your taskbar. I don't know how JAWS influences the alt-tab behaviour because this is in fact a Windows keystroke, but I think that somehow it does. Try unloading and loading JAWS again and see if your windows are back to normal. It usually works for me.
