I've used Google Forms and Sheets to gather responses, and then exported the data to Microsoft Excel. It is my understanding that over the past year, there have been significant improvements with the accessibility of Google apps. I have not confirmed this. They now offer e-mail support for accessibility. I'm not able to put my hands on that e-mail information right now, but if you can't find it, let the list know.
We do use Survey Monkey at work, but I have not been successful with being the creator. I'm not sure if it is user error or lack of accessibility.
