To add to Brian's message: Microsoft's new plug-in to save as PDF seems to work fairly well. I have not put it through it's paces yet. It claims to create accessible documents. It does have a few options, such as create PDF bookmarks automatically using either Word bookmarks or headings. I was surprised, however, that the Word styles like "Heading level 1" and so on were not recognized by JAWS in the Acrobat Reader as headings. I am glad to see Microsoft move in this direction! They also seem to allow saving in Open Office format.
