I have had slightly better results using Firefox to set up a channel
lineup.  This site often uses controls that do not work exactly like
JAWS expects.  Many times, JAWS will indicate it is an edit field.  Most
of the time, press Space to get into Forms Mode, then you can arrow up
and down in a pop up list.  Hitting Tab or Enter seems to work best to
select the desired item.  Custom lineups work the same, but in addition
you can just select your favorite channels from a standard lineup. 
Therefore, I first add a lineup for DirecTV - Philly, then add a custom
lineup selecting from that lineup. 

After hitting Enter on the "Add lineup" button, it goes to page 1 of 2
for setup.  By selecting the image labeled Satellite, you should not get
Time Warner on the second page where you are taken.  The second page
will ask for a zip code.  Once entered, a list box of providers will
appear.  Press Insert + Escape to make sure the JAWS info is updated. 
You should be able to arrow down to other options in your area like
Dish, DirecTV, and C Band.  

You will probably want to go to the "Manage Channel lineup" page to name
this lineup, create one or more custom lineups, and select the default
lineup.
