EdSharp works reliably. It isn't a substitue for Emacs or Vim under Linux (both excellent editing environments), but it's designed to be very accessible to screen reader users.
I am also interested in text editing options. Under Mac OS I use TextMate; under Linux, for serious work, I use Emacs, but Vim is useful for simpler tasks such as quickly editing configuration files or writing e-mail messages. For Microsoft Windows, I am not aware of anything that is both very accessible and very good (e.g., providing integration with revision control tools such as Git, which I use regularly). Git for Windows is accessible and works quite well. It comes with a Bash shell and basic tools, which are installed along with Git itself. Ssh is also included.
