I am also using Eclipse 3.3 with jbpm-jpdl-designer-3.1.2. I click a task node, 
then in the properties view, click the tasks tab. I click a task defined there, 
and then the Details tab. When I click Generate Form..., none of the form 
fields or buttons are populated with the process variables that I specified 
using the default controller.

I'm a little confused regarding the history of this item. I checked JIRA as you 
suggested, which implies [1] that the form generation feature has not been 
completed. I note that this item was logged in July 2006 and wonder if it is 
still relevant or I am simply misunderstanding something.

What confuses me is others seem to be using this functionality. For example, in 
a chapter from "Business Process Management with JBoss jBPM" available at [2], 
the author clearly generated some forms using the designer.

Does anyone know if this item is planning to be fixed in a future release...or 
is it already working for others in a different configuration than I am using?
