I'm having similar issues with the 403 error. But I did notice one issue with your application-policy.
This: <module-option name="java.naming.security.principal">cn=Manager,dc=mycompany,dc=com/module-option> Should be this: <module-option name="java.naming.security.principal">cn=Manager,dc=mycompany,dc=com</module-option> Not sure if that is your total problem but I would think not having a correct closing tag would casue some issue(s).
