Also you say you have generated the forms with the Designer, but the screen shots you attached are from https://github.com/droolsjbpm/jbpm/tree/master/jbpm-gwt/jbpm-gwt-form/src/main/resources https://github.com/droolsjbpm/jbpm/tree/master/jbpm-gwt/jbpm-gwt-form/src/main/resources, so you must have uploaded them to Guvnor manually?
