Thanks for the version information. Would it be possible for you to download the 4.0.4 RC1 JBoss application server and see if it happens. Significant changes for farmed deployments went into releases 4.0.3 + 4.0.4 but I don't think they would solve the problem (at least not directly) that you are seeing.
