A new revision of JEP 0038 is out now.

It deals with packaging graphic and sound files along with a definition
file for easy exchange and sharing between any supporting clients, used
for emoticons and ascii art within messages.

Changes: really just simplified and streamlined many parts of it.
Avoided many of the generic ideas, leaving those for a future spec that
is not so IM-specific. Please read through the spec in detail, and
report back any errors or questions you have. And this is really what I
envision the final spec to be like, so everyone can feel free to begin
making refernce implimentations and converting their code and existing
icon styles over to the updated spec.

Please expand on what the http://jabber.org/protocol/icon-styles namespace is to be used for. Does it serve a purpose other than telling clients you support this specification? (Is that even necessary?)

I see you still have not added my suggestion of an <x/> element in the <message/> with the name and version of the emoticon style that was used to compose the message body. Can you please tell me why not? It works, and has been doing a perfect job in my client since june.
Such a system would also be the first step in building the infrastructure needed to look up and retrieve unknown emoticon styles.
