Attached is a modified version of
gnome-themes-01-accessibility-icons.diff, which installs high and low
contrast versions of gnome-main-menu.png (the Launch menu icon), amongst
others.  The name of this icon changed to start-here.png in GNOME 2.16,
hence the modified patch.  The tarball containing the icons installed by
this patch has also correspondingly changed, and is attached.
  
I apologise for the binary crud in the patch (which is why I couldn't
just paste it inline), which is caused by its need to remove the
gnome-main-menu.png files before replacing them with start-here.png
files.  I'm not aware of any nicer way to make a patch do this; please
let me know if you are :)

FWIW, the other icons installed by this patch, as well the
gnome-main-menu -> start-here change, are now all taken care of
upstream, so this patch will be obsoleted when we upgrade to either
gnome-themes 2.16.2 or 2.17.3.

(The modification to gnome-themes-01 also required a minor adjustment to
gnome-themes-02, to keep it applying cleanly, but I presume I don't need
to bother you with that...)
