Luke Robertson commented on JENKINS-19907 Cannot use multiple parameters with parameter type equal to Radio Buttons This is due to the fields all have the name="value" but can be fixed following the Structured Form Submission guidelines (https://wiki.jenkins-ci.org/display/Jenkins/structured+form+submission) where the the name can be appended with a string which is removed during submission.
