First fix initializes the entryIndex during refresh. Second fix is in addEntry and adds entry to entryIndex with portlet name as the key (rather than object itself). >From my testing, this particular patch will allow for persistent addition and removal of portlets. Any new portlets are added to local-portlets.xreg (this file must exist or add/remove will fail).
