[FIX] db2 ddl  - unique constraints with nullable columns

What worked for me was to remove the language and country unique constraints and
define media_type and page as NOT NULL.  For my situation this was an acceptable
level of db-guaranteed uniqueness.  (USER_NAME->MEDIA_TYPE->PAGE) will always be
unique as USER_NAME (as well as ROLE_NAME and GROUP_NAME) are all forced to be
unique by application logic.
