HELP: create new UserException:ORA-02289

I run within Ora92 & win2k sp3 box, when I create new user, jetspeed report:

There has been an Error!
Reason: 
        org.apache.jetspeed.services.security.UserException: Failed to create account 
'test2': ORA-02289
