Are you by any chance running JDK 1.4.2 ? There are some XML parser issues with it, as JDK 1.4.2 comes with an integrated parser. See http://nagoya.apache.org/bugzilla/show_bug.cgi?id=21131, I've just added a comment from a user that may have a solution, please see if it works for you.
