I don't believe it is entered by hand.  $Id: $ is resolved when a cvs
commit/co occurs.  I don't think the author format has been discussed.  I'm

You can get this information from cvs or cvsweb.  Under linux/unix there is
a tool called rcs2log which will generate a changelog file directly from the
repository.  So if there is a changelog file I'd remove it from cvs and use
the commit comments directly from cvs to determine what happened.  I believe
there is a mailing list to get email every time someone commits.  The email
also contains the diff of the commit.  So I think there is enough places for
people to get information about what is changing.
