Thanks Karen, Actually, I never did find them. I have a box of papers that are filed & I was sure I had filed this application in there, so I could find it when I needed it. But I went through EVERY FILE thinking maybe it just got stuck in the wrong place by accident. I even went through the stack of towering papers on my desk & floor. But its ok. Dh said I can get a new application at an office down the street.
