Hi Michal,

Your configuration seems ok. 

What NET-SNMP version are you using?

Try executing: 

/usr/local/bin/php -q /usr/local/share/snmp/jffnms-snmp.php 
tc .1.3.6.1.4.1.2021.5001 debug

An OID tree should be on the output with all the information the script knows.
