I'm afraid I cannot add many details - the system is a remote server, it has a RAID1 on two SATA drives, is perhaps booted once a year, it does have at least one large and frequently exercised directory structure. The one message I saw during fsck also pointed to this structure. I have another eight similar boxes, maybe the same issue will hit one of the others.
