don's instructions should work in Win7. In case it helps, David Whitehead posted instructions that are slightly different and that might work for you.
Whitehead. You may want is to set the theme to classic, thus taking the Desktop out of the alt+tab cycle. Go to start menu, you will hear, search box edit, Type in, theme, you then will hear; Change the theme press enter. Now you have landed in the list of themes, The trick is now to find the classic theme. You will need to use your arrow keys. Once you hear jaws announce classic theme, press enter. You should hear a sound signifying the change. Now alt+f4. For the record, I recall reading that there's a drawback to choosing the classics theme in Win7, but I can't recall what it is. Whatever it is, it's the reason I didn't switch.
