Ann, It is possible to configure windows 8.1 so the key that used to bring up the start menu now brings up the all programs list. In reference to your question about the desktop, it is important to understand that Microsoft wants to lead people away from having large numbers of items on the desktop. This is because many elements on the desktop could make it difficult to use touch screen devices, such as phone and tablets running windows. Instead, Microsoft would like people to put frequently used programs on the task bar or to go to the all programs list. Frequently used programs automatically can end up on the task bar. End users can permanently pin an item to the task bar as well.
Bookshare has some excellent resources on windows 8.1, including a book written by David Pogue, who used to be the chief technology writer for the New York Times and had a large following.
