Ok, I see your problem--and your solution. I don't know about the
freezing part though. What I'm going to describe is very unconventional!
When you get to the first list box, if you'll open the listbox (it
doesn't say anything when I press Alt Down Arrow) then turn off forms
mode and use the regular virtual cursor, you'll hear all of the months.
Find the month you want and press Spacebar.
For the next list box, do the same. Open the listbox, then turn off
forms mode and use the virtual cursor to read the options. Press the
spacebar on the one you want to select Female, Male, or Other. (Yes, the
third option is Other.)
Mine didn't stick or freeze after that, so maybe the trick is not being
in Forms mode. I'm in IE11, but with JAWS 15. For what it's worth,
Google things tend to work a little better for me in Firefox.
