Hi Harry,
Thanks for your input. I believe the error was caused because the Microsoft
Speech form did not have the files loaded when you tried the forms out.
It is true that there are instructions on the page itself describing the
steps to go through for using the forms. In fact there are two types of
forms. One uses Microsoft Speech, that works along with JAWS 3.x, and the
other is simply using the JAWS drivers.
The Microsoft speech samples are the Purchase order form, a Michigan Court
form, and a F C C form. For these forms to work, it is required that you
load the 2 files mentioned in the instructions.
The JAWS form samples consist of a Leave Request Form, and a Budget Form. If
you already have JAWS 3.x running then the forms will work fine.
I probably was not clear in the email, because it is mentioned in the
instructions that when the form is loading the first time it will take a few
minutes while the form controls are getting installed, thereafter the forms
will come up quickly.
I am interested in your input, and look forward to having you try the forms
again. If you wish to go over the process more directly you may call me at
888-786-6455, ext. 227. I am on Eastern time.

Thanks Harry.
