When metrics reporters are reconfigured, remove the exporter or receiver registered by the old reporter from `ClientTelemetryExporterPlugin`. Otherwise, the plugin stays non-empty, telemetry RPCs remain enabled, and old exporters can accumulate. Use `CopyOnWriteArrayList` since exports may run while reporters are being reconfigured, and add tests for both exporter and receiver removal.
