The publish all function is a "best effort" process. This means that if a page
is locked or has validation errors when the user presses the publish all
button, then these pages will not be published.
Could you please check that the pages that do not get published are either
locked or have a validation error (missing mandatory field, missing title,
etc.) ?
Also check that there is not a problem with the workflow state image. Indeed,
it might be possible that the page is published, but the workflow state image
shown is the wrong one.
