Try changing a working soap message and then running the test

By command line I mean when you start Jmeter there are command 
line arguments to specify the proxy -H and port -P
I think you should perhaps pick these up as defaults or just use 
them and have no entry fields in the sampler.

No I am not getting double. The Loop count was set to 2
