Version 1.9.1 of JMeter only supports a single file in __CSVRead(). There were also some threading problems in CSVRead() in that release.
These was fixed a month or two ago in the nightly builds. If you don't want to upgrade, consider using _StringFromFile() instead. However, if you are using a recent build, and it is still causing problems, please create a Bugzilla report with full details and attach a sample test plan.
