Try enabling Debug on the Cookie Manager.

Select the Cookie Manager and use the Help Menu to enable it.

If the jmeter log file does not then provide a clue:

Can you post the contents of the Set-Cookie header?
[remove the value and name first!]
and
What is the URL to which you expect the cookie to be sent?
