Very often this type of error occurs if the file access rights are not 
set properly for the apache user, either for one of the directories in 
the path or for the file itself.

On a web server it might also be possible that there are defined 
redirection rules that in the end point to a false location.

You can check this if you try to access all relevant files explicitly 
either by a web browser or by a command-line tool like 'wget' or 'curl'.
