In a single JSP page, I am unsure how to combine the code for instantiating
a bean and calling a controller servlet. Specifically, I don't know how to
get the JSP to gather the information entered by a user (in the html form
fields) to instantiate a bean, and then when the user clicks the Submit
button, have the JSP call the controller servlet.

How do I do this, or is my logic flawed.
