Thanks Martin, I am fairly new to this area - but have done a lot of analysis on our mapping data recently. As I mentioned, it is happening only in a handful of scenarios. In the meanwhile, I switched our code to using the Topology Preserving simplifier which again has a handful of failures, but those ones get detected by calling the isValid method on the simplified geometry.
