Oh, I *do* have a passionate opinion about it :)

Title case, 'Usage:', makes more sense in docs, and is also the most
common for help text (I think it is in
the GNU style guide), so I think we have a winner. I will make the
changes. I will also
try and improved the presentation style of some of the common elements
(e.g. subcommands),
but that won't require changing anything in core.
