Thanks Doug!

My best guess is that when Stef installed RISE, since RISE has a dependency
on notebook >=4.2, it also updated their notebook to the latest available.
And since the extensions does not work with the latest one, is breaking...
Stef, you can always go back to the previous state using conda revision and
leaving the things in custom,js as they were. Or reinstall the previous
notebook version and install the extension as you installed before. Or wait
a little bit for Doug ;-)
