The letters are used when you have the option to show the sender's photograph set, but you don't have a photograph for that sender. The letter is the first letter of the sender's name, and K9 tries to use a different colour for each sender. It's all configurable under Settings > Global Settings > Display > Message Lists. You can also choose whether to "Show Stars" in the same place.
