You won't be asked if you (accidentally or otherwise) indicated "always" when prompted. You can clear the default by doing to system/apps/chrome (in this case) and select "clear defaults" toward the bottom of the page.
The ".net::ERR_CACHE_MISS" issue seems to point to a permissions issue. Izik, your phone isn't rooted per chance and/or you haven't been mucking (directly or indirectly) with the permissions that an app can have, have you?
