There's a db per account and a db_att per account. I don't know off hand what K-9 will do if an account's database will do if an account that exists db is deleted. I don't think it will re-download attachments on command.
We can look into why databases relating to accounts that don't exist are being kept and whether we are cleaning up old attachments. As for internal storage I don't know what is going on there.
