The "always show images" wording may be a bit confusing. I have found that images that are included in the message (and have been downloaded) are always displayed, and the "always show" is about fetching images that are referenced in html mail but not included. This is optional because it's a security issue, letting the sender know that you displayed the message and from what IP address.
