I've never been able to reproduce this issue here.
Do you create a new database every time?
Are you perhaps using any cleanup tools that delete the database after
uninstalling digiKam?
Do you have an antivirus program that fully scans a newly installed program
during its first launch?
I think you'll have to try to find the problem yourself.
A DebugView log from the first startup would be helpful.
