I have been able to test with a new HDMI monitor and cable and I do not see
this issue, so I'll resolve as "worksforme" and share my findings.

It appears that my original "ASUS Designo Curve Monitor MX34VQ" monitor would
advertise a 99.98 Hz refresh rate that neither the monitor or cable could
support (any longer I suppose).  So when kwin set up things for
plasma-login-manager, nothing appeared on the screen.

I didn't see this issue with SDDM, which just set the refresh rate to 59.97 Hz.
 While likely a hardware issue in my case that's solved by the new monitor and
cable, I'm not sure there's enough resiliency in plasma-login-manager yet -- to
go from a system that worked on SDDM to one that didn't using
plasma-login-manager was a surprise.
