There are some perfectly plausible scenarios :

For example,  you're deep in thought, staring at your half-finished code
window. You  figure things out and you're about to hit Enter  to to coding at
new line. Right before you press Enter , another process pops up a dialog . 

Since you haven't touched anything while thinking, anti-focus-stealing measures
won't kick in. The dialog appears out of nowhere, and gets focused, and guess
what? Your Enter key gets sent straight to that dialog instead.
