KWin already remembers the arrangement per set of outputs in ~/.config/kwinoutputconfig.json; for my laptop + external setup the stored position is bottom-aligned (3840,1080). Yet "Extend to left/right" via Meta+P always overwrites it with y=0 (common/osdaction.cpp, setPos({width, 0})). Attached patch: if the screen being placed is already adjacent and overlaps vertically, "Extend" keeps its y instead of resetting it to 0; otherwise nothing changes. This respects whatever the user arranged (bottom-aligned, centered, …) without changing the default, so it also fits the different preferences in this thread.
