It may not occur with any observable rhyme or reason yet, although considering that it's been occurring every few months, across nearly 7 installations, and it last *appeared* to occur when I invoked Dolphin, after having renamed a file via Firefox's File Picker portal, that *might* be a cause. I've put-off reporting this for years. I thought better to file a report, in case someone later discovers how to reproduce it.
